چکیده

Hospital Physician January 2009 23 Hyponatremia represents an abnormal ratio of total body sodium to water and is commonly defined as a plasma sodium concentration less than 135 mEq/L (1 mEq/L = 1 mmol/L).1–7 Hyponatremia is the most common electrolyte abnormality in hospitalized patients, with mild hyponatremia (plasma sodium < 135 mEq/L) present in approximately 15% to 22% of patients, moderate hyponatremia (< 130 mEq/L) present in 1% to 7% of patients, and severe hyponatremia (< 120 mEq/L) present in less than 1% of patients.8,9 Several populations are at increased risk of developing hyponatremia, including intensive care unit, postoperative, psychiatric, elderly, and nursing home patients.10–14 This article reviews the diagnostic and management approach to hyponatremia.

Fluid and electrolyte disorders are very common in critically ill patients. There are some circumstances in which a patient is placed in intensive care for the management of a specific electrolyte disturbance such as hyponatremia or hyperkalemia. The development of electrolyte disturbances among critically ill patients is also common due to breakdown of homeostatic mechanisms that prevent the d...

Electrolyte disorders associated with cancer.

Patients with malignancies commonly experience abnormalities in serum electrolytes, including hyponatremia, hypokalemia, hyperkalemia, hypophosphatemia, and hypercalcemia. In many cases, the causes of these electolyte disturbances are due to common etiologies not unique to the underlying cancer. However, at other times, these electrolyte disorders signal the presence of paraneoplastic processes...

A Rare Mechanism of Hyponatremia in HIV Disease

BACKGROUND Hyponatremia is the most common disorder of body fluid and electrolyte balance in clinical practice. It is associated with increased morbidity, mortality, and length of hospital stay. Little is known about the relationship between hyponatremia and HIV disease. It is thought that hyponatremia in HIV is associated with a syndrome of inappropriate ADH secretion (SIADH), volume depletion...

Vasopressin antagonists: role in the management of hyponatremia.

Hyponatremia is a common electrolyte disorder associated with potentially serious or life-threatening consequences. Serum osmolality and sodium concentration [Na+] are regulated by thirst, the hormone arginine vasopressin (AVP), and renal water and sodium handling. Hyponatremia is frequently caused by dysregulation of AVP, which accompanies disorders of water retention, such as congestive heart...
